B. The calculation used to arrive at the uncollectible amount of surcharge;

C. The total surcharge;

D. The month and year for which surcharge is remitted;

E. The legal name of company and telephone number and, if applicable, the parent company name, address and telephone number; and

F. The preparer's name and telephone number.

SUMMARY

This bill amends the law establishing emergency E-9-1-1 assessment procedures to clarify that prepaid wireless telephone service is subject to the same monthly assessment as other telecommunications service. Under the procedure established by the bill, an individual customer that purchases, in the State, a prepaid wireless telephone or a card or other device that allows additional use of a previously purchased prepaid wireless telephone shall pay the 50¢ per month assessment by one of 2 methods, as determined by the provider. Under the first method, the customer shall at the time of purchase pay 50¢ per month for each month in which the telephone or card may be used by that customer. Under the 2nd method, the customer shall pay 50¢ per month at the beginning of each month for which time is left on the service.
