Amend the bill by striking out the title and substituting the following:
‘An Act To Provide Access to State Forms’
Amend the bill by striking out everything after the enacting clause and before the summary and inserting the following:
‘Sec. 1. 5 MRSA §58 is enacted to read:
§ 58. Access to forms
Every state agency, department, board, office, commission, institution, authority or public instrumentality that requires filing of information by the public shall make a paper copy of any required filing form available, upon request, by regular mail at no cost to the requestor.’

This amendment, which is the majority report of the committee, strikes the provisions of the bill and instead requires all state entities to provide forms by regular mail at no cost to the requestor.
