United States Department of Health and Human Services, Center for Disease
Control and Prevention, to parents of a child excepted from the immunization
requirements under Title 20-A, §6355 and to sign a statement that the
information has been provided. |

local activities in this bill represent a State mandate pursuant to the
Constitution of Maine. General Fund
appropriations are provided to fund 90% of the additional costs. |

This bill
establishes the Vaccine Information for Parents account within the Department
of Education and provides a one-time General Fund appropriation of $3,332 in
fiscal year 2013-14 and ongoing General Fund appropriations of $768 per year
beginning in fiscal year 2014-15 to the account for the purpose of paying 90%
of the total cost to local school administrative units to assemble and send
the required information that school health advisors must send to
parents. This fiscal note is based on
