Unless the transaction is exempt under section 172, the owner of residential real property shall provide to the purchaser a property disclosure statement containing the following:

1.__Disclosure.__Disclosure by the owner of the following information:

A.__The type of system used to supply water to the property.__ If the property has a private water supply, the owner shall disclose:

(1)__The type of system;

(2)__The location of the system;

(3)__Any malfunctions of the system;

(4)__The date of the most recent water test; and

(5)__Whether the owner has experienced a problem such as an unsatisfactory water test or a water test with notations;

B.__The type and location of insulation used on the property;

C.__The type of waste disposal system used on the property.__ If the property has a private waste disposal system, the owner shall disclose:

(1)__The type of system;

(2)__The size and type of the tank;

(3)__The location of the tank;

(4)__Any malfunctions of the tank;

(5)__The date of installation of the tank;

(6)__The location of the leach field;

(7)__Any malfunctions of the leach field;

(8)__The date of installation of the leach field;

(9)__The date of the most recent servicing of the system; and
