Sec. 1. 22 MRSA §1596, sub-§2, as repealed and replaced by PL 1989, c. 274, §1, is repealed and the following enacted in its place:

2.__Abortion reports.__A physician performing an abortion shall file a written report with the department on forms provided by the department.

A.__The abortion reporting form must contain the following information:

(1)__The mother's medical record number;

(2)__The mother's age;

(3)__The date of the mother's last menstrual period;

(4)__The clinical estimate of fetal age, based on examination, ultrasound or both;

(5)__The code number for mother's county of residence.__ The department shall assign and retain the list of county codes;

(6)__The code number for physician.__The department shall assign and retain the list of physician codes;

(7)__The month, day and year the abortion was performed;

(8)__Which of the following abortion procedures was the primary procedure used:

(a)__Medication, the name of which must be provided;

(b)__Suction curettage;

(c)__Dilation and evacuation;

(d)__Intact dilation and extraction;

(e)__Other dilation and extraction;

(f)__Instillation procedure;

(g)__Hysterectomy or hysterotomy; or

(h)__Other;
