(1)__State that the student is required to attend school pursuant to section 5001-A;

(2)__Explain the parent's right to inspect the student's attendance records, attendance coordinator's reports and principal's report;

(3)__Explain that the failure to send the student to school and maintain the student in regular attendance is a Class E crime in accordance with section 5053-A and explain the possible penalties;

(4)__State that the superintendent may notify the local law enforcement department of a violation of this statute; and

(5)__Require the parent and the student to attend a conference with a designated official.

D.__Prior to notifying the local law enforcement department under paragraph E, the superintendent shall schedule the conference required in paragraph C, subparagraph (5).

E.__If, within 3 days after receipt of the notice referred to in paragraph C, the child remains truant and the parent and child refuse to attend the conference according to paragraph D, the superintendent shall report the facts of the unlawful absence to the local law enforcement department, which may proceed with a criminal action against the parent unless the child is at once placed in an appropriate school.

F.__When a student is determined habitually truant and in violation of section 5001-A and the superintendent has made a good-faith attempt to meet the requirements of paragraph B, the superintendent may notify the local law enforcement department of the child's truancy.__After this notification, a local law enforcement officer who sees a truant may transport the truant to the appropriate school if the truant:

(1)__Is off school grounds during school hours; and

(2)__Is not under the supervision of school personnel.

3.__Reports.__The following provisions apply to reports of habitual truancy.
