C.__Whether the applicant meets the requirements for
licensure under this chapter.

 
This requirement includes taking whatever action is necessary to
permit the Chief of the State Police or a designee to have access
to confidential records held by banks, the courts, law
enforcement agencies and the military for the purposes stated in
this subsection.

 
4.__Investigations; national criminal history record check.
Any individual applying for a license, any partner of a
partnership or any officer, director or holder of any ownership
interest of a corporation or association applying for a license
as a licensee, manufacturer or wholesaler must submit to a
background investigation by the Chief of the State Police to
verify the applicant's compliance with the requirements of
section 371, subsection 2.__In order to determine the eligibility
of an applicant for a license under this chapter, the Chief of
the State Police shall require the applicant to furnish a full
set of fingerprints to enable a criminal background investigation
to be conducted.__The Chief of the State Police shall submit the
fingerprints to the Federal Bureau of Investigation for a
national criminal history record check.

 
5.__Notification of municipal officers.__An applicant for a
license to operate a video gaming terminal must send a copy of
the application to the officers of the municipality or
unincorporated place in which the terminal will be operated.__The
applicant must certify in the application that the copy has been
sent and must list the names and addresses of the persons to whom
the copy was sent.

 
§376.__Fees; term of license; nontransferable

 
1.__License levels; fees.__The annual fee for a license issued
under this chapter is as follows:

 
A.__A license for a video gaming terminal manufacturer is a
Level 1 license and the fee for a video gaming terminal
manufacturer license is $3,500;

 
B.__A license for a video gaming terminal wholesaler is a
Level 1 license and the fee for a video gaming terminal
wholesaler license is $3,500; and

 
C.__A license to operate a video gaming terminal is a Level
2 license and the fee for a video gaming terminal operator
license is $500 per terminal.
