C.__The shipment must be conspicuously labeled as follows:__
"CONTAINS ALCOHOLIC BEVERAGES.__SIGNATURE OF AN ADULT 21 YEARS
OF AGE OR OLDER REQUIRED FOR DELIVERY."

 
D.__Each shipment must include an invoice containing the
name, address and phone number of the recipient and the
source of the verification of the recipient's age.

 
E.__The out-of-state shipper shall ensure that shipments
into this State are made by a duly licensed carrier and that
the carrier obtains the signature of an adult at least 21
years of age to acknowledge receipt.

 
4.__Filing and record keeping; jurisdiction.__An out-of-state
shipper licensed pursuant to subsection 2 shall:

 
A.__File with the bureau on a quarterly basis on forms
supplied by the bureau:

 
(1)__A verified report of all malt liquor or wine
shipped into this State during the preceding quarter;
and

 
(2)__The excise tax imposed by section 1652 and the
premium imposed by section 1703 on all malt liquor and
wine shipped into this State as shown on the report
filed pursuant to this paragraph;

 
B.__Maintain copies of the reports filed pursuant to
paragraph A for at least 3 years and any accompanying proof
that allows the bureau to determine the accuracy of the
reports;

 
C.__Submit to an audit upon a reasonable request of the
bureau; and

 
D.__Consent to the personal jurisdiction of the bureau or
any other agency of this State and to the courts of this
State concerning enforcement of this section and any related
laws or rules.

 
5. Renewal of out-of-state shipper's license.__An out-of-state
shipper shall annually renew the out-of-state shipper's license
by:

 
A.__Filing a renewal request with the bureau.__The shipper
shall include a true copy of:
